The appendix has been split to demonstrate the lumen. On the external surface there is considerable congestion in the distal two thirds of the appendix and the mesentery is somewhat thickened. The peritoneal surface of the appendix is shaggy indicating the inflammatory reaction.
On section numerous diverticula are present on the mesenteric border of the appendix. The mucosa which in the main is somewhat oedematous and thickened can be seen to dip down through the muscularis on the mesenteric border and form small sacs, globular in outline in the substance of the mesentery. There is no obvious retention of contents within these and they extend from the tip of the appendix for approximately two thirds of the organ.
In view of the manner in which the diverticula are formed only of mucosa it is clear that these are acquired and not congenital lesions.
