- Colon showing diverticular disease with a coincidental carcinoma From a male aged 84 year The patient complained of weakness and rectal bleeding for some months. Early large bowel obstruction The specimen is a resection of sigmoid colon measuring approximately 18cm in length. The salient feature is the presence of diverticular disease which is more readily seen on the mucosal aspect. A fairly marked hypertrophy of the muscularis is associated focally. There is a loss of mural architecture, probably resulting from inflammatory change. near one margin of excision is a fungating annular lesion of neoplatic appearance. The specimen thus exhibits both diverticular disease and a coincidental carcinoma Microscopical examination The mucosa shows a marked polypoid structure with pouch-like formations dipping deeply into the muscle coats - a diverticulitis picture. there is also a considerable malignant area of glandular type infiltrating both muscle coats, but not extending into the subserosa. Theres is much congestion with thickened, dilated blood vessels and subserosal oedema
